Functional and structural imaging of migraine revealed alterations of the pain-modulating system, including the anterior cingulate cortex (ACC) which has an important role in top-down control of subcortical areas. The increased activation of ACC has been observed in healthy subjects during serotoninergic challenge. However, it has been unknown so far whether serotonergic control of ACC activation is altered in migraine.

In this study we investigated how serotoninergic challenge influences activation pattern of ACC in migraine patients compared to controls.

Changes in brain activation following 7.5 mg i.v. citalopram infusion, were measured in 27 healthy and 6 migraine volunteers using within-subject, placebo-controlled, double-blind, randomized design. The volunteers took part a 30 minutes-long challenge pharmacological fMRI (phMRI) two times, placebo and citalopram respectively. These occasions were separated by at least two weeks. The phMRI images were acquired at a 3T MRI scanner (Philips Achieva) using a T2*-weighted echo-planar sequence. Imaging data were analysed using SPM 12 for MATLAB.

The changes in ACC activation over time during and after citalopram infusion were obtained by time x drug interaction F-test (primary uncorrected p=0.001 with secondary Family Wise Error corrected p=0.05). Migraineurs showed significantly different ACC activation compared to controls. Two clusters were observed in the right ACC (p(FWE)=0.006, 9 voxels and p(FWE)=0.022, 6 voxels) where the extracted time-series indicated that activation of ACC in migraine patients was significantly higher in the first 8 minutes of infusion than in healthy controls.

These results clearly demonstrate that migraineurs are more sensitive to alterations of serotoninergic tone, since they have increased ACC activation compared to the control group especially in the first few minutes of drug response. The altered response to citalopram in migraine patients may be the result of imbalance between ACC and other pain modulating areas that may contribute to repeated migraine attacks.
