Revascularisation of the human mucosa after vestibuloplasty procedures

Objectives: The apically repositioned split thickness flap (ARSTF) combined with a collagen matrix (CM) is a surgical procedure to enlarge keratinized gingiva. Aim of the present study was to define blood flow patterns, which might characterize the wound healing of such surgical procedures during the first 6 months of healing.
Methods: Five patients with inadequate width of keratinized gingiva in the anterior mandible were treated by vestibuloplasty with the use of ARSTF combined with a CM (Geistlich Mucograft). Intraoral photographs and Laser Speckle Contrast Imaging blood flow (BF) measurements were taken at baseline and at 16 follow-up visits for 6 months. Thirty regions of interest of the augmented mucosa (AM) and the surgically unaffected surrounding mucosa (SM) were evaluated, BF values were analyzed in relation to intraoral photographs. Data were statistically evaluated by Mixed Model approach.
Results: At 6 months, mean width of keratinized gingiva measured 3.8±0.28 mm compared to 1,4±0.29 mm at baseline. Scar formation was observed within all augmented cases. Four-month BF remained unchanged until the end of the follow-up period in both AM and SM, thus these values were defined as baseline (BSL, 100%). BF was measured at each visit as percentage of BSL. In the first 5 days post-surgical ischemia was observed in AM (BF 47%, p<0.01), surrounded by hyperemic SM (BF 182%, p<0.01). From day 6, BF increased in AM, with a hyperemic peak on day 21 (BF 140%, p<0.01), followed by a decline toward baseline. At 6 months, BF at non-cicatrized grafted tissues was lower compared to BSL in SM, whereas cicatrized grafted tissues showed even lower values.
Conclusions: Vestibuloplasty using ARSTF combined with a CM resulted in substantial gains of keratinized tissue width. Postoperative BF at 6 months was inferior compared to the adjacent mucosa, and low BF might relate to scar tissue formation.
