Serum uric acid level is independently associated with subclinical atherosclerosis in asymptomatic population

Introduction: Searching for early atherosclerosis at subclinical stages is an important step of cardiovascular prevention. Carotid ultrasound and heart CT scan are sensitive non-invasive tools for detecting subclinical atherosclerosis. Higher serum uric acid levels are known to be associated with cardiovascular diseases. However, there is some inconsistence regarding the information on the independence of the association.
Aim: The aim of our study was to assess the prevalence of subclinical atherosclerosis and its associations with SUA and conventional risk factors in an asymptomatic population. Methods: A voluntary cardiovascular screening program was performed with 2420 participants. Participants with a history of cardiovascular disease were excluded from further analysis. Parts of the screening were health questionnaire, physical examination, carotid ultrasound, laboratory tests and CT scan as a sub-study.
Results: Mean CIMT max was 0.75 ±0.14 mm. CIMT max was above 0.9 mm in 270 cases (17.9%). A Total Agatston score of 0 was found in 33.1%. The score was above 300 in 15.5%. Median score was 17,8 (136). SUA showed positive correlation with both CIMT (0.289, p <0.01) and total Ca-score (0.257, p<0.01). Logistic regression analysis adjusted for conventional risk factors revealed that SUA level is independent predisposing factor for both CIMT max>0.9 (OR: 1.002, p<0.001) and total Ca-score>300 (OR: 1.01, p<0.001).
Conclusion: In our study, association of SUA with subclinical atherosclerosis was still present after the effects of conventional risk factors were eliminated.
