Introduction
Streptococcus pneumoniae can cause severe infections mainly in children ≤5 years and in the elderly. Asymptomatic carriers - especially children - are major reservoirs of infection. Conjugated vaccines (PCVs) have an effect on both pneumococcal diseases and carriage, containing 7/10/13 serotypes from the known 94. PCV7 became recommended in 2009 in Hungary; it was later replaced by PCV13, which is mandatory since 2014.
Aims
Our aim was to demonstrate vaccine efficacy both in carriers and in patients, and to monitor serotype replacement after the implementation of PCVs in Hungary.
Methods
Clinical isolates (n=155) derived from the central laboratory of Semmelweis University, carried isolates (n=206) from screening 522 children attending nurseries, between 03-2015 and 05-2016. Serotyping was based on latex agglutination and PCR, the antibiotic sensitivity was measured by agar dilution. Group 1 comprised the carriers (n=206, <7 years), group 2 patients aged <7y (n=64) and group 3 patients ≥7y (n=91).
Results
PCV13 coverage was 7.8%, 17.2% and 30.8% in group 1, 2 and 3, respectively. In group 1, only 19A, 19F and 9V represented the PCV13 serotypes. In group 2, an elevated number of serotype 3 was observed next to 19A and 19F, meanwhile in group 3, serotype 3 was predominant. Regarding antibiotic sensitivity, higher MIC values were measured for clinical strains (e.g. levofloxacin resistance was 0,0%, 6,3% and 16,5% in group 1, 2, and 3).
Conclusion
In the surely vaccinated people (groups 1 and 2), the effect of PCVs is obvious, and PCV13 coverage in group 3 is still lower than that of invasive cases in 2015 (58%). We can conclude that serotype replacement is continuing, but fortunately these non-vaccine types are sensitive to antibiotics. The previously threatening multiresistant 19A strains show decreasing tendency and other PCV13 serotypes (19F, 3) are present mainly in older people.
