Introduction:
Type 2 diabetes mellitus (T2DM), gestational diabetes (GDM) and non-alcoholic fatty liver disease (NAFLD) are among the most prevalent metabolic diseases, especially in the advanced countries. There is a clinically significant association between T2DM and NAFLD, in addition women with a history of GDM are more prone to develop T2DM later in life.
Aims:
Our aim was to investigate the association between the intrahepatic lipid content (IHLC) the glycemic parameters, and the PNPLA3 risk genotype (G/G).
Method:
26 volunteers (mean age=36.73 +/- 4.17 years) were enrolled to the study: 15 subjects were with and 11 without history of GDM. Allele-specific PCR (KASP, LGC Genomics) was used for genotyping for PNPLA3 rs738409 SNP. Ten patients with G/G and 16 with C/C genotype were studied. 75g OGTT (fasting, 30’ and 120’ plasma glucose [PG]) and HbA1c measurements were performed to assess the glycemic state. IHLC was measured using 1HMR spectroscopy and chemical shift imaging. Non-parametric statistical tests (MWU and SRO) were used.
Results:
IGT was diagnosed in 6 individuals at OGTT (pGDM/control 4/2). IHLC was significantly higher in individuals homozygous for the PNPLA3 minor allele (G/G) compared to those with C/C genotype (means=11.73% vs. 3.51%, respectively; MWU-test: p=0.025).
Significant correlations were found between IHLC and OGTT PG values in PNPLA3 genotype defined subgroups: G/G: r=0.80/0.80 for 0’/120’min PG values (p<0.05); C/C: r=0.61/0.55 for 0’/120’min PG values, respectively. Correlations between IHLC and BMI (r=0.72, p<0.05) were also significant. There was no difference in PG and HbA1c values according to PNPLA3 rs738409 genotype.
Conclusion:
PNPLA3 (G/G) homozygosity substantially increases the IHLC in a metabolically benign manner. IHLC displays a positive correlation with PG values at OGTT, when analyzed in separated subgroups according to PNPLA3 genotypes, suggesting that additional - metabolically active - factors should contribute to the net IHLC.
