Introduction: According to an international systematic review, 14.4% of surgical patients have experienced at least one AE during their treatment and 5.2% resulted in potentially preventable AE to the patient.
Aims: BELLA (Accreditation of Healthcare Providers for Safe Supply in Healthcare) program is interlinked with the safety of surgical patients in several ways. The purpose of the current BELLA study is to create a model which not only explores the circumstances and effects of the occurrence of AEs but measures their economic impact as well.
Method: Using retrospective methodology the samples were randomly selected from past cases of a surgery department. These samples were reviewed using a two stage review process by three experienced physicians. Firstly, the AE suspicious cases were identified and assorted using an inquiry form containing predetermined selection criteria. Secondly, all the medical records of these suspicious cases were reviewed independently by two physicians and finally, the decision whether the AE occurred was made by consensus.
Result: The study sample examined 264 cases, among them 29 patients experienced 40 AEs. AE group length of stay (LOS) in the surgical department was 14.13 days, while LOS of non-AE group was only 4.3 days; the yearly average LOS in the participating department was 4.91 days. Main causes of AEs included surgical procedures (47%), therapeutic errors (25%), deficiency in nursery care (10%) and other procedures (15%). AE patients went through an additional 45 antibiotic therapies and 72 medical examinations during their treatment. Due to the consequences of AEs 8 operations were performed. 28 of 40 AE have found highly preventable.
Conclusion: This methodology seems reliable to estimating AEs, however the inquiry form requires simplifying adjustments. The initial cause of AEs is remained yet to be explored and analysed. The economic implications of AEs have to be evaluated.
