Emotion recognition in schizophrenia: an ERP-study

Introduction: Facial emotion recognition is a complex social function, including early visual processing, structural encoding and late cognitive process. Impaired social functioning (e.g. emotion recognition) is a core feautre of schizophrenia and it leads to long-therm disability. The exact neurobiological background of the deficit in emotion recognition in schizophrenia is still unclear.
Aim: This study aimed to explore the characteristics of event-related potentials evoked by facial emotion recognition in patients with schizophrenia.
Method: In this case-control study we analyzed the high density (128-channel) EEG data of 18 patient with schizophrenia and 15 healthy controls. The two groups were matched by gender, age, and education. During the Emotion recognition task 80 happy, 80 neutral and 80 sad faces of the ’Karolinska Directed Emotional Faces (KDEF)’ set were presented to the participants. Subjects were asked to identify the emotions by button press. The investigated event-related potentials were P100, N170 and N250, focusing on the parieto-occipital, temporal and frontal brain regions. Average evoked potential waveforms were computed from epochs for each stimulus type and for both groups.
Results: The study groups differ significantly (p <0.05) in the recognition of happy, neutral and sad facial expression. Schizophrenic patients showed significantly (p <0.05) decreased P100 amplitude in happy and sad condiction in the parieto-occipital regions. There was no significant difference between groups in the N170 component. We found marginally significant (p <0.1) difference in N250 component over the fronto-central regions.
Conclusion: According to our findings schizophrenic patients showed impaired early sensory perception (P100) and relatively intact structural encoding (N170) of the face. This deficit in early sensory perception may lead to higher level disfunctions, such as emotion recognition, witch further contribute to the psychosocial functioning deficit.
