Gingival blood flow measurement reproducibility by Laser Speckle Contrast Imaging method

Introduction: Laser Speckle Contrast Imaging (LSCI) method is a promising tool for non invasively measuring human blood flow. However, reproducibility of blood flow readings in oral mucosa is unknown.
Primary aim was to investigate factors affecting reproducibility, such as angulation, retraction of soft tissues or using reflected image via mirror.
Methods: Healthy participants were involved in this study. In the test first series (n=22) six snapshots were taken from perpendicular to the gingiva and with angulation from left and right. Each were repeated. In the second test series (n=22) three snapshots were taken, patients closed their mouth between the measurements and the lips retraction was carried out with dental mirrors. These series were repeated one week later for inter day reproducibility. In the third series (n=26) six snapshots were taken either by direct view or by mirror. Obtained data was statistically analyzed by Linear Mixed Model.
Results: The pairwised comparison showed that the microcirculation in the left turn was significantly higher in means than in the central control position (3.6%, p<0.05). No significant changes were observed in means between repeats (1.6%p=0.287). Repeatability of subjects were found to be excellent (intraclass correlation coefficient=0.91) with good variation value coefficients (5.5% [4.6%-6.7%]) of the repeated measurements. The angulation caused low (2.1%[1.2% 3.8%]) and non significant variation within a subject. Lip retraction did not affect mean blood flow values, coefficient of variation was (8.3% [5.9%-11.5%])(p<0.001), while in the case of inter day reproducibility was (6.4% [3.8%-10.8%])(p<0.05). Using a mirror did not affect average blood flow value, but slightly (5.9% [3.8%-9.2%] p<0.05) influenced the repeatability.
Conclusions: LSCI has excellent short time reliability, regardless lip retraction or an indirect view. Although angulation influences measurements, this slight difference has to be taken in consideration. LSCI also seems to be appropriate for comparing measurements made on different days.
