Text of the abstract

Introduction: The rate of osteoporotic fractures increases as the population ages. In Hungary, 14% of men and 40% of women over 50 suffered osteoporotic fractures. Hip fractures account for 63-72% of all fracture-related hospital admissions in patients over 50. An international research which observed the quality of hip-fracture care showed that in Hungary outcomes are significantly lower than in other countries. While 30-day mortality rates of hip fracture decreased during the observed years, 1-year mortality remained unchanged and high. This result suggests that exploring the underlying results is necessary. It is hypothesised that healthcare outcomes are influenced by the characteristics of the healthcare system as well as by the social care system and socio-economic circumstances.
Aims: The aim of the study is to explore what factors influence the quality of hip-fracture care and what causes lead to high mortality rates.
Methods: A literature review and semi-structured interviews with professionals will provide information as to what factors should be included in the analysis of the quality of hip-fracture care. This will serve as the methodological foundation of further analyses. Data for the analysis will be available from national databases.
Results: The result of our research will be the complete set of factors to be considered and the final methodological plan of the study regarding the quality of hip-fracture care. Our initial research suggests that the main influencing factors are the accessibility to acute, rehabilitation and primary care, quality and process of rehabilitation, funding policies, socio-economic background (education and financial status).
Conclusion: The regression analyses exploring the reasons behind the high mortality rates will be performed taking the above mentioned factors and those which will be defined by the literature review and interviews into consideration.
