Introduction:
Cardiovascular risk plays a major role in the long term mortality and morbidity, in pediatric kidney transplantation (KT) and should be monitorized well. However serious CV events are rare in children, early signs of pathological processes are important in the long-term survival. 24-Hour Ambulatory Blood Pressure Monitoring (ABPM) is the gold standard in blood pressure (BP) profiling.

Aims:
The main purpose of the study was to describe the BP profiles and changes, 5 and 10 years after KT.

Methods:
86 pediatric kidney recipients (mean age: 11,26 (± 3,95 years; 52 males) were retrospectively analysed. We investigated 110 ABPM measurements 1,02 (± 0,65) years (Timepoint A), 5,05 (± 1,05) years (Timepoint B) and 10,41 (± 1,78) years (Timepoint C) after transplantation. We also collected casual blood pressure (CBP) measurements, demographic-, anthropometric-, donor-, complications-, medication informations and outcomes.

Results:
14 rejections and 1 exitus occured, 4 out of them in our timerange. Mean 24-hour BP were A: 86,24 ± 8,65 Hgmm B: 86,75 ± 10,12 Hgmm C: 87,88 ± 7,94 Hgmm. Non-dippers and reverse-dippers (n) were A: 24 and 8 (76,2%), B: 29-6 (66,04%), C: 20-5 (78,13%). Normal tension was observed in incresing numbers over the years. White coat hipertension (HT) was observed significantly higher earlier after Tx. Masked HT decreased over time and sustained HT were stagnated. Controlled HT increased and non controlled HT decreased during 10 years.

Conclusion:
BP profile did not considerably change during the 10 year timerange, although more and more patient had controlled HT with medication. BP-dipping stagnated over time and it’s role as a risk factor may need further evaluation in our study. However, white coat HT was remarkably high at timepoint A, but decreased over time.
