Introduction: The incidence of acute otitis media is approx. 11%. At least 5% of these patients develop a chronic condition, leading to around 31 million new chronic otitis media cases annually. Clinical presentation of the disease is diverse. A common factor in the patomechanism is the impairment of the ventillation of the tympanic cavity. Ventillation disorder of the middle ear is most frequently caused by the ventillation problems of the Eustachian tube. The only broadly available causal treatment of the above is balloon dilation (BD), yet its’ mechanism of action has been unknown.
Aim: Examination of the local effects of balloon dilation on the structures of the Eustacian tube.
Methods: BD was carried out on 6 unfixed Eustachian tubes in cadavers where post mortem time was less than 24 hours. Control specimens were gained with the anatomical preparation of 30 Eustachian tubes extracted from cadavers fixed in PFA and according to Thiel. Dilation was performed during 2 minutes on 10 bar pressure. Following fixation of these specimens, morphologic analysis took place and dilated samples were compared to the control specimens.
Results: In each case BD dilated the lumen through its entire length. Local mucosal and submucosal destruction could be identified. The cartilage of the auditory tube at the bend was cracked (fractured) at the luminal surface. The injury of the cartilage was located to the middle third of the auditory tube in each case.
Conclusion: High pressure destruction injures the endoluminal epithelium and the submucosal connective tissue. The balloon, despite its constant diameter, has different effects on the different parts of the cone-shaped Eustacian tube. Cartilage injury develops in the middle third. Our results suggest that the patomechanical background of the therapeutic effect of BD can be the healing that follows this acute destruction.
