The Effect of the Onset Order of Comorbidity between Migraine and Depression on Physical and Mental Health Status

Introduction The association between migraine and depression is well-known, but an overlooked component of this comorbidity is the temporal order of disease onsets. Some suggest that anxiety disorders tend to prelude, while depression tends to develop after migraine onset, but a bidirectional migraine-depression connection seems to be the best fitting model.
Aims Our goal was to investigate the effect of the temporal order of onsets of migraine and depression on the physical and mental health status of groups with different disease trajectories.
Method A sample of the UK Biobank database (Application no. 71718) was used (n=387014; 52.9% female, mean age: 56,11 years (SD: 8.06)). Based on International Classification of Diseases (ICD) codes (F32/33 for depression, G43 for migraine), the following groups were compared: first migraine, then depression (M-D, n=2480), first depression, then migraine (D-M, n=1493), only migraine (M, n=15973), only depression (D, n=41142) and controls (C, n=325926). Phenotypic data included years in education, total household income, body fat percentage (BFP), smoking status, alcohol intake frequency, overall health rating, sleeping problems, social support and activity, neuroticism, current depression status and recent stressful life events. Statistical analyses were implemented in SPSS 27.
Results D-M and D cases showed the lowest years in education (H=578.193, p<0.001), income (H=4774.828, p<0.001) and frequency of confidential relationships (H=1641.722, p<0.001). BFP was the highest among comorbid cases (F=1451.668, p<0.001), while smoking in the D group (H=1327.767, p<0.001) and alcohol intake in the C group (H=2832.047, p<0.001). The D-M group reported the worst health status (H=10808.42, p<0.001) and the highest level of sleeping problems (H=4782.238, p<0.001), neuroticism (F=8634.715, p<0.001), current depression (F=8963.894, p<0.001) and stressful life events (H=4067.332, p<0.001).
Conclusion Our results suggest that the first depression, then migraine group shows the highest level of physical and mental health symptoms and this group is exposed to a stronger disease burden even among comorbid cases.
