Introduction: Currently, a significant number of migraine patients cannot take advantage of pharmacotherapy for various reasons. Since stress is a major trigger of migraines, non-pharmacological treatment, such as headache-specific autogenic training (AT), could help patients reduce their headache frequency.
Aims: To evaluate changes in brain activation patterns underlying emotion processing in migraineurs compared to healthy controls, as well as changes in headache frequency in the migraine subsample after regular practice of AT.
Methods: Episodic migraineurs without aura and healthy controls participated in a 16-week long AT, based on the stress-relieving Schultz-type autogenic training and supplemented with psychoeducation, movement therapy and pleasant imagery. Functional magnetic resonance imaging measured subjects’ brain response to emotional faces before and after AT (migraineurs were scanned in the headache-free phase). Also, the migraine frequency was assessed in the migraine group before and after AT.
Results: After the training course, deactivation of the migraine-associated dorsal pons was found in migraineurs using fearful stimuli. Activation changes of the supplementary motor area elicited by fearful faces and insula elicited by happy faces differed significantly in migraineurs compared to healthy controls. Migraine subjects reported significantly fewer headache attacks after AT.
Conclusion: Our results show that the headache-specific autogenic training can contribute to improvements in migraineurs’ clinical parameters and induce neural changes in emotional processing regions that have an important role also in migraine generation and pain modulation. Therefore, AT can be an effective adjuvant to pharmacotherapy of migraine.
