Three months oral iron supplementation does not improve performance in young basketball players: a randomized placebo-controlled study

Introduction: Athletes have higher iron demand due to increased iron loss, insufficient intake and absorption in connection with physical burden. Although iron deficiency is one of the most common factors that may reduce sports performance, supplementation forms and doses are still not standardized in athletes. Our aim was to assess iron status of young basketball players and to study the effect of iron supplementation in a randomized placebo-controlled study.
Methods: During a detailed sports cardiology screening, questionnaire, ECG, laboratory tests including iron panel, echocardiography and cardiopulmonary exercise test (CPET) with lactate measurements were performed. Low iron status was defined as a ferritin level lower than 100 ug/L. Athletes with low ferritin were randomized into two groups: iron-group and placebo-group. Oral iron supplementation (iron (II) sulfate) or placebo (Vitamin C 50 mg) was administered for 3 months and all exams were repeated. Results: We included 65 (age 15.8±1.7 years) youth male basketball players in 4 age groups (U15, U16, U18, U19). Laboratory tests did not verify anemia and there was no difference in hemoglobin and hematocrit levels between age groups. Prelatent iron deficiency was observed in 60 (92%) athletes. Parallel to the increase of ferritin level with aging (U15: 32.3±24.6 ug/L vs. U19: 63.2±22.9 ug/L, p<0.005), total iron binding capacity decreased. The CPET results proved the expected performance improvement with training and age. Performance (U15: 258.2±52.9 Watt vs. U19: 360.6±52.2 Watt, p<0.05), maximal aerobic capacity (U15: 3.2±0.6 L/min vs. U19: 4.2±0.6 L/min, p<0.001) and ventilation (U15: 119.5±23.3 L/min vs. U19: 150.3±30.0 L/min, p<0.05) increased with age. After 3 months supplementation, ferritin levels were significantly higher in the iron-group compared to the placebo-group (75.5±25.9 ug/L vs. 54,9±10.4 ug/L, p<0.01). Ferritin > 100 ug/L level was achieved only in 15% of athletes. There was no difference in sport performance parameters between the iron-group and the placebo-group (VO2 max: 4.0±0.5 L/min vs. 4.2±0.8 L/min, p=0.38). Conclusion: Ferritin levels increased due to iron supplementation, however, only a small portion of athletes achieved the expected ferritin level after the 3 months supplementation. Therefore performance improvement could not be detected in our athletes.
