Introduction: Patch test is the standard diagnostic tool for verifing contact hypersensitivity in the daily dermatological allergology practice. Not only delayed-type, but immediate-type contact reactions can be observed as well. Patch test is commonly indicated in several dermatoses, thus confirming the underlying contact allergy in the backgound of the skin diseases.

Aims: Our study aimed to analyse the clinical characteristics of our patch tested patient population with at least one verified immediate-type contact reaction.

Method: Patients were patch tested in the Allergy Outpatient Unit of Department of Dermatology, Venereology and Dermatooncology of Semmelweis University between 2007-2020. The European Environmental Baseline Series (EEBS) and the Complementary Fragrance Series (CFS) allergens were used. The tests were made in accordance with international methodological standards for patch tests in 48-hour occlusion. Evaluation of the skin reactions happened in 20-60 minutes, 48, 72, 96 hours and on the 7th day.

Results: During the 14-year period 5543 patients were patch tested with the EEBS and 3124 with the CFS at the same time. Among them 384 patients had at least one immediate-type positivity. Female predominace was observed (81.8%). Most patients belonged to the age group of 41-50 (24.0%). 50.8% of the patients had one, 35.7% had two, and 13.5% had three or more immediate-type contact reactions. The most common contact allergens were benzoic acid (86.2%), sorbic acid (38.8%), and balsam of Peru (30.2%). Regarding delayed-type co-hypersensitivities nickel, fragrances, and preservatives can be mentioned.

Conclusion: In case of typical anamnestic features, when immediate-type contact reactions are suspected, patch tests are advised to perform. Patients can be educated about the different sources of allergen exposures after identifing the provoking ones, which should be avoided lifelong in order to prevent future allergic symptoms.
