Introduction: Intraductal papillary mucinous neoplasia (IPMN) is the most common cystic lesion of the pancreas. The clinical significance of it is given by its relative asymptomaticity and high malignant potential. Recently, the effect of the immunosuppressive stromal microenvironment in tumor progression became known in different types of tumors. This special microenvironment can be caused by the NLRP3 inflammasome activation and tumor-infiltrating lymphocytes. Aims: The aim of the present research is to determine the effect of the effector molecules in the inflammatory signaling pathway on survival and the oncopathological properties of IPMN. Methods: In our study, we examined histological specimens from patients diagnosed with IPMN at the Department of Pathology, Forensics and Insurance Medicine between 2008 and 2018 (n = 70). Tissue microarrays (TMA) were prepared from the samples and labeled by immunohistochemistry using antibodies to IL-1β, IL-18, IL-1Ra, caspase-1, and CD4, and sections were digitized and evaluated by H-Score and cell counting. Patient data were collected from the institute's internal database (MedRec), the electronic health record system of Semmelweis University, and the database of the Central Statistical Office. Results: Patient survival reduced in the presence of elevated protein expression of IL-1β (p = 0.05), IL-18 (p = 0.05), IL-1Ra (p = 0.000019). In invasive cases, we observed significantly higher IL-1β (p = 0.013) and a tendency to higher IL-18 and IL-1Ra expression. Lower IL-18 levels were measured in mixed ductal involvement (p = 0.043) and intestinal subtype (p = 0.03). Lower CD4+ cell infiltration was present with lymphovascular invasion (p = 0.03). Examining the correlation of these proteins, it can be said that the expression of IL-1β, IL-18, IL-1RA, as well as the lymphocyte density and the expression of IL-18 and caspase-1 showed a significant positive correlation. Conclusion: The differential expression of effector molecules in the inflammasome signaling pathway has prognostic and may have therapeutic value for the patients with IPMN. Determination of IL-1Ra expression can be used as a strong and sensitive negative prognostic factor.
