Colorectal cancer (CRC) is one of the most common cancers in our country. Approximately, half of the colorectal cancer patients acquire metastatic lesions to their liver (CRCLM) leading to a poor-prognosis disease. While surgery may be an effective treatment for primary cancers, metastatic cancers require systemic treatment such as immunotherapy. It seems like lymphocytes that infiltrate the vicinity of tumors tend to overdrive inflammasome activation resulting in the production of certain IL1 family member pro-inflammatory cytokines. Physiologically this is a self limited process, but in case of tumors reaches pathologic level.
We assume that inflammasome components or products of its activity could serve as predictive biomarkers correlating metastatic capacity and clinical outcome thus finding these markers could be of immense help to pathologists during staging of CRC and mCRC in a more reliable way. In this study we investigate inflammasome activation in intact colon mucosa, CRC and CRCLM. Moreover, the possible correlation of inflammasome activation and different clinicopathological factors will be determined.
After forming tissue microarrays of formalin fixed paraffin embedded blocks, the identification of potential biomarkers will be carried out with consecutive immunohistochemical staining, expression levels will be determined via the H-score, thus results can easily be implemented into routine diagnostic procedures in the field of pathology even in case of fine needle aspiration biopsies where the amount of samples is limited.
Inflammasome activity certainly occurs in colorectal cancer and colorectal cancer liver metastases. Just as in the case of primary tumor T stage, the IL-1β producing part of inflammasome activity showed higher intensities in case of high-grade tumors, whereas IL-18 production (also a result of inflammasome activity) was higher in low-grade tumors. A highly significant correlation between the normal colon tissue and the primary tumor or liver metastases was found in all inflammasome related markers.
Taking into account the relative amount and activity of the two major inflammasome effectors has to be considered with respect to tumor stage, tumor grade and primary tumor localisation, as these can serve valuable information about the antitumorigenic and protumorigenic effects of the inflammasome activity in the given context.
