Magnetic resonance imaging analysis of artefacts caused by different biliary stents

Introduction: Biliary stent implantation has an important role in the treatment protocol of patients with advanced pancreatic ductal adenocarcinoma (PDAC). The implanted stents may cause different types of artefacts on magnetic resonance images (MRI), which impedes adequate treatment follow-up assessment.
Aims: In this study, our research group investigated the possible causes of these artefacts.
Methods: In this retrospective single center study, PDAC patients with 3 different types of implanted biliary stents (uncovered nitinol, covered nitinol and covered platinol) were included and MRI data was obtained according to standard clinical protocol. T1 and T2-maps were acquired at 1.5 T. Subjective image quality was evaluated on a 5-grade Likert scale, size of artefact was measured on T2-maps, stent alignment to the magnetic field and luminal air presence were also assessed. Correlation was determined between the size of the artefacts, the image quality, influencing factors and different stent types.
Results: 31 patients with PDAC and implanted biliary stent were included retrospectively. No correlation was found between the size of artefact, the subjective image quality and type of stent used. Moderate correlation (τ=0.41 p < 0.05) was found between air presence in the surrounding bowel structures and size of the artefact and between the stent-magnetic field angle size and subjective image quality (τ=0.47 p < 0.05).
Conclusion: Since the stent position and therefore the angulation to the magnetic field is unchangeable, the best-available pre-acquisition protocol solution to prevent stent artefacts is to reduce the amount of air in the surrounding bowel structures. Our study team has already initiated a prospective study to assess the effectivity of dimethicone administration in preventing imaging artefacts.
