Results of complex cardiopulmonary rehabilitation for post-COVID patients

Introduction: After COVID-19 infection, some of those who have recovered may experience persistent symptoms such as cough, dyspnoea, chest pain, shortness of breath, fatigue, anxiety, depression, whether mild or severe infection was caused by the coronavirus. Almost half of those affected (44%) also experienced a lasting deterioration in quality of life.
Aim: Our study aimed to evaluate the effectiveness of complex cardiopulmonary rehabilitation and to assess quality of life and functional parameters before and after 10 days of rehabilitation.
Methods: At the start of the rehabilitation programme at the Department of Pulmonology, Semmelweis University, we assessed respiratory function (FEV1ref%), 6-minute walk distance (6MWD), chest wall expansion (CK), quality of life (EuroQol-5D, mMRC) and functional status after Covid-19 (PCFS). The individualised rehabilitation programme included controlled breathing techniques, chest wall mobilisation, respiratory and peripheral muscle strengthening, endurance training on bicycle and treadmill, dietary and psychological assessment and counselling.
Results: 68 patients have participated in the programme, avarage age: 54 years (48-63), male/female ratio: 39/29.The rehabilitation programme has resulted in significant improvements: 6MWD (m): 492 (435-547) vs. 523 (477-580); p=0.031; mMRC: 1 (0.25-1) vs. 0 (0-1); p=0.003, EQ-VAS score: 75 (65-80) vs. 85 (75-90); p=0.015, PCFS: 1 (1-2) vs. 0.5 (0-1); p=0.032. Respiratory function and chest kinematics scores also improved: FEV1 (ref%): 88 (78-100) vs. 92 (85-99); p=0.188; CK (cm): 3.5 (2.75-4.25) vs. 4 (1-5.25); p=0.296, breath holding test (BHT): 33 (23-44) vs. 41 (28-58); p=0.041.
Conclusion: Complex cardiopulmonary rehabilitation tailored to the individual is beneficial and recommended for all symptomatic post-COVID patients. It improves patients' quality of life, physical exercise capacity and reduces their symptoms. Overall, the clinical condition of patients improves.
