Introduction: The incidence of cutaneous melanoma shows a significant increase worldwide and is in the background of the majority of skin-cancer related mortality. The most important prognostic factor of melanoma is the Breslow tumor thickness that is provided by the histopathological evaluation of the excised tumor. The optimal surgical safety margin for melanoma excision is also based on the Breslow thickness that is only available before the surgery. Thus, often a reoperation is required if the primary surgical margin was not sufficient or in other cases, it reveals that a larger surgical margin was given than necessary. In this work, we aimed to compare two non-invasive imaging techniques, multispectral imaging (MSI) and high-frequency ultrasound (HFUS) for the preoperative prediction of the Breslow tumor thickness of melanoma.
Materials and methods: We carried out MSI using 405 nm (autofluorescence), 525 nm (green), 660 nm (red), and 940 nm (infrared) channels and 33 MHz high-frequency ultrasound imaging with optical guidance of primary cutaneous melanomas. Two subgroups with great clinical relevance were established to categorize the investigated melanomas based on the Breslow tumor thickness (<1 mm and >1 mm).
Results: We included 39 patients with primary cutaneous melanomas with a mean age of 65 ±12.40 years. The sex ratio was 46% female and 54% male. The mean histological Breslow tumor thickness was 1.928 ±1.589 mm. The intensity of the IR channel of the MSI had the strongest correlation with Breslow thickness (r:-0.678), thus, this was used to differentiate between the two subgroups. The sensitivity and specificity of the MSI were 78.9% and 85% and those of the HFUS were 89.5% and 100%, respectively. Both techniques showed a strong significant correlation with the Breslow tumor thickness (MSI: r: -0.698, p: <0.0001, 95% CI: -0.38306 to -0.4905, HFUS: r: 0.8881, p: <0.0001, 95% CI: 0,7955 to 0,9402).
Conclusions: In our study, in comparison with MSI, HFUS showed a stronger correlation with histological Breslow thickness. Further clinical studies are needed to investigate how these novel non-invasive techniques may aid the clinicians in the preoperative prediction of Breslow thickness in order to determine the appropriate surgical margins and required staging procedures for cutaneous melanoma.
