Assessment of Peripheral Pulse Wave Analysis in Estimating Cardiac Ejection Time

Introduction: Ultrasound examination of the heart is of great importance in the diagnosis and monitoring of certain cardiac conditions. Analysis of the peripheral pulse wave recorded by photoplethysmography (PPG) provides parameters which may also reflect alterations in cardiac function. The ejection time (ET) parameter measured by echocardiography is used to evaluate left ventricular contractility and pump function. The relationship between the PPG and the echocardiography parameters has not yet been established, but comparable values can be obtained by performing these two tests simultaneously.
Aims: The aim of our study was to analyze the correlation of various PPG parameters with echocardiographic ET in healthy subjects.
Method: 28 healthy volunteers (M/F: 10/18; mean age: 35,89 ± 13,08SD) who were not diagnosed and treated for cardiovascular or other chronic disease, not smoking, not regular alcohol drinkers, not pregnant, BMI between 18-26, and not having a SARS-CoV-2 infection in the past 6 months were included. The measurements were carried out at SE, Városmajor Heart and Vascular Clinic. After giving their informed consent in writing, volunteers were asked about medical history/lifestyle, and blood pressure was measured. Then the echocardiography scan was performed and simultaneously a 2-minute pulse wave was recorded in the supine position using a pulse oximeter, placed on the right index finger. Using an automated Matlab-based software analysis, 16 PPG parameters were calculated and compared with echocardiographic ET using Pearson correlation.
Results: From the calculated PPG parameters, the parameters that showed significant correlation with echocardiographic ET parameter included ‘Cardiac Ejection Time’ (r=0.784), ‘Bottom-to-Peak Time’ (r=0.779), ‘ELVET1’ (r=0.673) and ‘ELVET2’ (r=0.740) (early left ventricular ejection time 1 and 2) (p<0.001 in each case).
Conclusion: Our results suggest that several PPG parameters can be used to assess the ET of the heart reliably. Therefore, home monitoring devices equipped with PPG analysis modules may provide valuable information for the diagnosis and treatment follow-up of heart failure patients. In the future, we plan to complement the study by including heart failure patients.
