Efficacy of Peppermint Essential Oil for Reducing the Symptoms of Nausea and Vomiting: Systematic Review and Meta-Analysis

Introduction
Nausea and vomiting are common symptoms that can occur in various situations, including postoperatively, as a result of chemotherapy, or during pregnancy. Pharmaceutical treatments for these symptoms are not always effective or have adverse effects. Peppermint oil could be a potential alternative option. There is no evidence-based knowledge about the efficacy of peppermint oil in treating nausea and vomiting symptoms and it needs further investigation.
Aims
To systematically evaluate the efficacy of peppermint oil in reducing the symptoms of nausea and vomiting during pregnancy (NVP), in postoperative nausea and vomiting (PONV) or in chemotherapy induced nausea and vomiting (CINV) compared to placebo in randomized controlled trials (RCTs).
Method
Systematic review and meta-analysis followed by the PRISMA guidelines using the PICO framework and registered in the PROSPERO. A systematic search of five databases was conducted in November 2022 to identify relevant publications. This study included placebo-controlled clinical trials involving patients who underwent surgery, were pregnant or received chemotherapy, and experienced nausea and vomiting symptoms. The risk of bias and quality of evidence were evaluated by RoB2 and GradePro. Statistical analysis was performed in R software.
Results
Three RCTs of PONV, including 155 postoperative patients, and three RCTs of NVP, including 182 pregnancy patients were pooled for statistical analysis. In the case of NVP, peppermint oil has a significant effect after four days based on various scales and questionnaires (standardized mean difference (SMD): -0.37, 95% confidence interval (CI): -0.68; -0.07). In the case of PONV, peppermint oil has a significant effect after 10 minutes based on various scales (SMD; 0.62, 95% CI: -1.07; -0.17).
Conclusion
Our systematic review and meta-analysis revealed that inhalation of peppermint oil had a beneficial effect on reducing nausea and vomiting symptoms in pregnant women. However, in the case of postoperative patients, peppermint inhalation did not show a considerable decrease in the severity of the symptoms.
