Introduction: The EQ-5D is one of the most commonly used generic preference-based health-related quality of life measures in the world, which is recommended by the health technology assessment guidelines in more than 20 countries, including Hungary. However, it may not be able to capture some gastrointestinal and extraintestinal symptoms related to celiac disease (CD), therefore additional dimensions ("bolt-on") may improve its validity.
Aim: To test the psychometric properties of the EQ-5D-5L and the following five bolt-ons: gastrointestinal problems (GI), cognition (CO), sleep (SL), tiredness (TI), and diet (DI).
Method: In 2020–2021, a cross-sectional online survey was conducted amongst 312 adult CD patients in Hungary. Patients completed the EQ-5D-5L questionnaire and the five bolt-ons, Gastrointestinal Symptom Rating Scale (GSRS) and the Satisfaction with Life Scale (SWLS) questionnaires. Ceiling effect reduction and linear regressions were used to explore the impact of bolt-ons on EQ Visual Analogue Scale (EQ VAS). Correlations between the five core dimensions and bolt-ons were computed. Known group validity was assessed by examining whether the EQ-5D-5L+bolt-ons were able to discriminate between different health statuses (poor-fair, good, very good-excellent), GSRS tertiles, and SWLS groups.
Results: The ceiling effect decreased from 38.8% to 7.4% when using the EQ-5D-5L+5 bolt-ons. TI led to the highest ceiling reduction (22% absolute reduction). Known-group comparisons confirmed that the bolt-on items improved the discriminatory power of the EQ-5D-5L: GI, TI and SL dimensions improved the discriminatory power by health status, while GI, TI, SL and DI dimensions for GSRS tertiles; and GI, DI, CO, and TI for the SWLS groups (p<0.001). Weak or moderate correlations were found between the bolt-ons and the five core dimensions, with the exception of GI that showed strong correlations with pain/discomfort (PD) (rs=0.51). The GI and CO bolt-ons improved the explanatory power of EQ-5D-5L on EQ VAS (the adjusted R2 increased from 0.41 to 0.44).
Conclusion: Evidence from our study suggests that adding bolt-ons may increase the validity of EQ-5D-5L of patients with CD. The GI and TI bolt-ons performed particularly well in multiple psychometric tests. Further research is needed to explore whether there is any overlap between the GI bolt-on and the PD dimension.
