A pilot feasibility study of the M3 Training: Enhancing readiness to change among adolescents with chronic pain

Introduction
Chronic pain (CP) is significantly prevalent among children and youth, and the occurrence is generally increasing with age. Adolescents with CP have significantly lower activity, are more sedentary, and have poorer physical functioning compared to healthy peers. Pain is always an embodied experience that can limit present and future physical actions and can detrimentally affect the way of how adolescent sampling the world and consequently how their experience their bodies. Motivational interviewing is one recommended way to understand, elicit, and enhance movement related change among patient with CP. In this context, however, it has never been tested in paediatric population.
Aims
The first aim of the presentation is to describe the development of the M3 Training and to evaluate the feasibility and acceptance of it. The second aim is to present preliminary data regarding the effects of our novel intervention on teens’ and parents’ readiness to change and self-efficacy.
Method
The study was a single arm, non-randomized pilot feasibility study. Adolescents (N=34) between 12-18 years, with CP more than 3 months, without ongoing organic disease, and their primary caregivers were included. Data was collected before the first M3 Training session and after the third session at 1 month. Feasibility was evaluated using enrolment, drop-out, retention, and adherence rates and training engagement. Acceptability was assessed by an open-ended question. Wilcoxon rank-sum test was used to assess the changes in outcomes between the two time points. Cohen’s d effect sizes were also calculated.
Results
The intervention’s acceptability was good. 82% of the participants finished the program and 74% accomplished fully or partially their movement-related goals. The responders reported that the program was helpful and useful. Statistically significant increase was in readiness ratings among adolescents and parents in maintenance scores with large effect size.
Conclusion
The acceptability and feasibility of the M3 Training suggest that it could be a promising approach to improving outcomes for adolescents with CP by addresses both the physical and psychological aspects of this condition. Further research in this area is needed to confirm the long-term effectiveness of the program on self-efficacy and other pain related outcomes.
