Thyroid Disturbances After SARS-CoV-2 Infection and the effect of COVID-19 Vaccination in Children

Introduction: Recent clinical data have raised awareness of the possible association between the novel coronavirus (SARS-CoV-2), vaccination against COVID-19 and thyroid disturbances. However, studies among the pediatric population are still limited.
Aims: Our aim was to assess the prevalence and permanency of thyroid autoimmunity (TA) and dysfunction diagnosed after SARS-CoV-2 infection in children with detailed analyses of possible risk factors such as vaccination.
Methods: Data were collected in a prospective registry of three pediatric Long COVID outpatient clinic at Semmelweis University from 24th March, 2021 to 23rd March, 2022. We collected the following parameters from all children regardless of their symptoms: sex, date of birth, history of previous thyroid disease, thyroid laboratory values (anti-thyroid peroxidase [ATPO], antithyroglobulin [ATG], thyroid-stimulating hormone [TSH], and free thyroxine [fT4] if the TSH was altered). TA rate among vaccinated and unvaccinated children was determined. Those with newly diagnosed alterations underwent thyroid ultrasound examinations and their follow-up was initiated.
Results: We enrolled 458 children with confirmed previous SARS-CoV-2 infection (mean±SD age: 12.4±3,8 years, 45.4% male). From further analysis we excluded six patients with a preexisting thyroid disorder. TA was found in 30 children (6.6%), TSH was altered in six of them. Isolated TSH abnormality (all elevated) was seen in an additional eight cases (1.8%). Ultrasound-proven autoimmune thyroiditis (AIT) was 4.0%. Of the 38 cases with a newly found thyroid alteration, 36 were followed for a mean±SD of 12.7± 4.3 months. Among children with TA, 73.3% had long-lasting disturbances. No association was found between COVID-19 vaccination and TA (χ2(1,N=452) = 0.138, p = 0.815).
Conclusion: We discovered a considerable rate (8.4%) of previously not diagnosed thyroid disturbances after COVID-19. To assess if these can be connected to SARS-CoV-2, further, controlled studies are needed. The majority of newly diagnosed alterations remained positive, therefore regular, long-term follow-up is crucial. Vaccination had no effect on the prevalence of TA.
