Hungarian Version of the Mainzer Audiometric Test for Children

Introduction
Early detection of hearing loss in children has an utmost importance due to consecutive impaired speech development. Although Hungarian speech audiometry is used for adults since 1960, there is no suitable test for pediatric testing.
Aims
Our scope was to adapt a well-established German speech audiometry test for children (MATCH) to Hungarian language.
Methods
The MATCH is a supra threshold, age-appropriate speech audiometric method, using 26 nouns. Previous studies have shown that these words are most likely part of the lexicon of children aged 2 years and older. The test is a picture-pointing task with a four-option non-forced choice method. First, we performed an online study among children aged 3-7 to survey the identification of each test picture by Hungarian children. We set up the vocabulary by a professional female voice, we recorded the words in Hungarian language. General requirements for the sound were provided by the ISO standard 8253-3: SNR at least 45dB, flat frequency response of recording system, soundproof studio. As source format we use PCM 16 Bit, 48kHz, mono. In the third part of the study, we performed the standardization studies on 30 normal hearing subjects. The validation studies are performed with 15 subjects having hearing loss. Each subject was tested previously with play audiometry, tympanometry, otoacoustic emissions and auditory steady-state response (ASSR) as well.
Results
An online study with 170 participants revealed a good recognition of the item pictures among Hungarian children aged 3-7 years. The relative frequency of the phonemes fitted well the distribution of the Hungarian adults’ spontaneous every-day language (BEA-database). The long-term average speech spectra of the recorded words showed a good correlation with the average speech spectra of female speakers. The intelligibility curves for the Hungarian words fitted the average in 22 of 24 words. The test showed a good usability, the average test time was 12-23 minutes. The measurement could be completed by all the 30 subjects. Some children showed a reduction of concentrated attention after 15 minutes. Therefore, we changed our protocol to take the tests in two separate visits. We experienced a superior acceptance of the test by children and their parents.
Discussion
During the adaptation we managed the difficulty, that the simple translation of the vocabulary did not correspond to the language specific phoneme distribution. Some words had to be changed to achieve a better phoneme arrangement. It was a challenge to find synonyms for the items, keeping the pictures unchanged with sufficient recognition at the same time. Special abilities of children must be considered at the establishing of pediatric testing protocols. We provide insight and point out some pitfalls in adapting a speech test to Hungarian language.
