Investigating the impact of obesity on postoperative complications in ulcerative colitis: Systematic review and meta-analysis

Background : Ulcerative colitis (UC) is an immune-mediated inflammatory bowel disease what’s prevalence is around 200/100,000 people per year. About 20% of patients will need to have a colectomy. 70-80% of these operations are elective. Approximately 1 in 2 adults with UC are overweight or obese. The effect of obesity on postoperative complications is uncertain.
Objective: Our aim is to compare the postoperative complications between obese and non-obese UC patients undergoing total proctocolectomy with ileal-anal pouch anastomosis.
Methods: In this systematic review and meta-analysis, we conduct our research in 3 databases on the 26th of November 2022. We have done the selection, the data extraction, and the risk of bias assessment by two independent reviewers. Any disagreement was resolved by a senior author. Retrospective, prospective cohort, and case-control studies are included. Case reports, case series, paediatric, in vitro or animal studies are excluded. For data analysis, we used mean difference (MD) for continuous and odds ratio (OR) for dichotomous results.
Results: We included 3 retrospective cohort studies for analyses covering a total of 5013 patients in our research. Two of these were propensity score matched. At the investigation of the length of hospital stay, the MD was 0.37 [95% CI -0.06; 0.8; p= 0.065]. This difference is neither clinically relevant nor statistically significant. In the occurrence of the septic and total complications in 30 days after surgery, the OR was 1.11 [95% CI 0.85; 1.46; p= 0.234] and 1.08 [95% CI 0.65; 1.79; p= 0.589]. Although the obtained differences don’t reach the level of statistical significance, we cannot rule out that it has a certain clinical relevance.
Discussion: Our findings show no significant difference between the two investigated groups, although does not exclude the opportunity of the effect on different types of complications. The low number of involved studies and the lack of data about different types of complications restricted this research. On the other hand, due to the increase of BMI in this population, it would be worth conducting the investigation with other BMI values.
Conclusion: According to our current information, obesity does not represent an increased risk for the development of the overall complication rate, but a more detailed examination is needed to identify minor differences.
