Introduction: Small cell lung cancer (SCLC) represents one of the most aggressive cancers. It is characterized by its tendency for early metastatic spread, particularly to regional lymph nodes. However, the molecular landscape of the primary tumor does not necessarily align with the expression of subtype- and therapy-related markers of corresponding lymph node metastases.

Aims: This study aimed to investigate the differences in molecular expression patterns between primary SCLC tumors and their lymph node metastases.

Methods: 32 metastatic SCLC patients who underwent surgical resections between 1978 and 2013 at the National Korányi Institute of Pulmonology with available tissue specimens were included in our study. The samples from both the primary tumor and the lymph node metastasis were evaluated by RNA expression analysis and IHC.

Result: When comparing the expression levels between primary tumors and their lymph node metastases, we found a significant difference in the expression of DLL3 (p=0.008) and NeuroD1 (p<0.001) in lymph node metastases compared to primary tumors based on the IHC staining. However, neither of these tendencies could be verified with RNAseq. When observing the molecular subtype of samples, we found a tendency for change between the primary tumor and its lymph node metastasis. The most common changes were from SCLC-A to either SCLC-AN or SCLC-QN and from SCLC-QN to SCLC-AN and SCLC-N due to tumoral evolution or intratumoral heterogeneity, respectively.

Conclusion: This study highlights the discordance in protein expression between primary tumors and corresponding lymph node metastases in SCLC. It also sheds light on the phenotype switching between various SCLC subtypes during the disease progression. The knowledge is beneficial for uncovering molecular mechanisms driving metastatic spread in SCLC and could influence the development of targeted therapies and prognostic markers.
