Introduction. Myocarditis is a polyetiological inflammatory heart disease with a wide range of clinical manifestations from asymptomatic to life-threatening conditions, including the probability of dilated cardiomyopathy (DC) development. Myocarditis is utterly underdiagnosed, and mechanisms of its chronicity and DC launching, especially in terms of sex differences, remain poorly understood.

The aim of this work was to investigate the immune response in the myocardium of male and female BALB/c mice at 14 days after autoimmune myocarditis modelling.

Materials and Methods. Myocarditis was modelled in adult male (n=10) and female (n=10) BALB/c mice by subcutaneous injection of 150 μg of α-myosin (n=5 of male and n=5 of female) with 120 μL of complete and incomplete Freund's adjuvant (Sigma) on days 0 and 7. In 14 days after the second immunization the evaluation of the mean number of immune cells and fibroblasts per field of view (25,000 µm), the ratio of collagen fibers of types I and III via picrosirius and Mallory staining, and pro- and anti-inflammatory cytokines mRNA expression by RT-PCR was performed.

Results. The mean number of immune cells and fibroblasts in the myocardium was higher in both experimental groups relative to corresponding control groups, which indicate the development of myocarditis. There were no differences in the volume of collagen fibers of types I and III, probably due to the early stage of disease. Upregulation of pro-inflammatory markers Nf-κb and Tnf-α were observed in both experimental groups relative to corresponding control groups. At the same time, Hif-1α and Il-1β expression levels were upregulated in females, but downregulated in males with myocarditis in comparison with controls. The dynamics of pro-inflammatory cytokine Il-6 and anti-inflammatory ones Il-10 and Tgf-β mRNA expression was the opposite.

Conclusion. Reported changes displayed different local immune response of female and male mice to early stage of myocarditis development. At this time, pro-inflammatory background was observed in both groups, but in males, it was more balanced with anti-inflammatory cytokines expression. These distinctions should be taken into account for therapy approaches and further investigations concerning CD development risks.
