Introduction: Lyme disease is the most common tick-borne disease in the U.S. and Europe. Disulfiram (DIS) worldwide is used in chronic alcoholism treatment as oral or, in the near past, implantable tablets, topically against head lice, flat lice, and scabies. It recently is gaining ground for Lyme disease treatment. However, due to poor oral bioavailability, the drug fails to achieve the translational goal in the new potential indication scope.
Aim: Given the significant clinical interest, the promising results from preclinical trials, and the off-label use of DIS, our team undertook a comprehensive systematic literature review. This review was crucial in evaluating the drug's current status in Lyme disease, emphasizing its clinical relevance and identifying its translational issues. Using pharmaceutical technology tools, a proposal composition and a non-invasively administrable pharmaceutical form was developed.
Methods: Cyclodextrin (CD) inclusion complexes were studied for solubility and stability enhancement. Solubility study and solid-state characterization of the inclusion complexes were assessed to determine the suitable combination. The developed formulation relates to a novel preparation for rectal administration comprising a mixture of DIS with random methylated-β-CD and hydroxypropyl-β-CD.
Results: The molecular encapsulation of the drug with CDs leads to improved solubility and, consequently, a more favorable bioavailability. The advantage of DIS suppository over peroral administration is that the rectal route is less susceptible to the first-pass effect. This, in turn, allows for a reduction in the required dose and a decrease in the risk of adverse effects, making it a potentially safer and more effective treatment option.
Conclusion: Combining the pharmaceutically relevant CDs and the already approved chemical entity is advantageous for drug repositioning strategies, optimizing the innovation triangle — basic research, applied research, and drug development. The traditional non-invasive rectal dosage form of DIS presents the advantage of production in laboratory pharmacies, fasting the availability of the drug for patients with Lyme disease.
