The role of Interleukin 1, Toll-like receptor 4 genes certain single nucleotide polymorphisms and detected bacteria in the prognosis of medication-related osteonecrosis of the jaw

Introduction: The medication-related osteonecrosis of the jaw (MRONJ) is the side effect of antiresorptive drugs in the maxillofacial region.
Aim: Examination of the Interleukin 1A, 1B (IL-1A, IL-1B), Toll-like receptor 4 (TLR-4) genes certain single nucleotide polymorphisms (SNP) and the roles of bacteria in the prognosis MRONJ.
Methods: The DentiGen Parodontitis Test was applied in collecting samples from oral mucosa cells for examination of IL-1A (rs1143634) and 1B (rs18000587) polymorphisms. The genetic samples were evaluated with DNA-hybridization technique. The samples were collected from venous blood to ascertain TLR-4 SNPs (rs4986790, rs4986791). The samples were analysed with the Sanger method. Microbiological samples were collected during the operation. The prognosis was registered following the treatment based on stage improvement, recovery and relapses.
Results: 79 patients, treated with surgical therapy, were enrolled in the examination of IL-1 polymorphisms. In this group, stage improvement was detected in 78, recovery in 67 and relapse in 33 cases. 49 of 79 patients had unfavourable allelic variants. No association was found between the examined polymorphism and the stage improvement (p=0,382) or recovery (p=0,561). A significant association (p=0,022) was detected between the relapses and the occurrence of unfavourable allelic variants.
60 genetic samples have been evaluated in the investigation of TLR-4 polymorphisms. Unfavourable allelic variants were detected only in 3 of 60 cases. In this group, stage improvement was registered in 59 cases, recovery in 54 cases, and relapses in 10 cases. Based on our preliminary results, no association was found between the examined polymorphism and the stage improvement (p=0,963) recovery (p=0,707) or relapses (p=0,627). A significant association (p=0,06) was detected between the number of relapses and the occurrence of unfavourable allelic variants.
Microbiological samples were collected in examined IL-1 and TLR-4 groups. The statistical analysis is in progress.
Conclusion: Based on our preliminary data, an association was found between relapses of MRONJ and the occurrence of the examined IL-1 polymorphisms. A correlation was found between TLR-4 SNPs and the number of relapses.
