Subjective and quantitative assessment and follow-up of olfactory performance in patients with PostCOVID olfactory complaints

Introduction: SARS-CoV-2 infection can be associated with long-term complications affecting quality of life, such as permanent impairment of olfactory function. Smell training has a positive effect on the improvement of olfaction.
Aim: Subjective and quantitative assessment of olfaction in patients with postCOVID olfactory complaints, follow-up of patients, evaluation of the effectiveness of olfactory training and investigation of influencing factors in a prospective study.
Methods: We studied 943 postCOVID patients with olfactory complaints, recorded their clinical parameters, and assessed olfactory performance (TDI) using the Sniffin' Sticks test at first examination and at follow-up 6 months later. Between the two occasions, patients were offered medicational treatment and olfactory training. During the follow-up, therapeutic adherence was assessed by questionnaire and subjective olfactory improvement was assessed.
Results: 943 postCOVID patients with olfactory complaints (285 male, 657 female, mean age 43 and 45 years) were studied. The mean TDI at baseline was 25.01 (SD: 8.69) for males and 24.46 (SD: 8.35) for females. 334 (male: 233, female: 101) patients were included in the control study. Patients who attended the control had an average baseline TDI of 22.53, which was significantly lower than the same value for the whole patient population (24.6). 252 patients completed at least 50% of the training, of which 188 completed at least 75%. Training at 75% or more increased patients' TDI score by 2.87 points over six months, compared to 1.8 for training below 25% (the difference is not significant due to the large variance in scores). A greater change in TDI is also associated with a subjective improvement in distorted sense of smell (p=0. 003). Olfactory training also had a positive effect on distorted sense of smell (parosmia). Patients with improving parosmia symptoms, followed training 81% on avarage, compared to the others whose symptoms did improve with 63% theraputic adherence (p < 0.001). Non-smokers were more than twice as likely to improve parosmia (OR: 2.19, 95% CI: 1.31-3.07).
Conclusion: Spontaneous improvement plays a role in the recovery of parosmia, and olfactory training at an appropriate frequency helps this process. Typically, those who start from a worsened state return to control. Smoking habits can affect the development of parosmia.
