Photon-Counting CT for the detection of obstructive coronary artery disease: a diagnostic accuracy analysis

Introduction: The novel photon counting detector computed tomography(PCD-CT) with increased spatial resolution, and reduced beam-hardening artifact created a new field in coronary stenosis assessment compared to the energy-integrating detector CT(EID-CT). Aims:In our study, we aimed to evaluate the diagnostic accuracy of PCD-CT compared to the EID-CT using invasive coronary angiography(ICA) as the reference standard. Methods: We enrolled patients with stable chest pain and≥50% stenosis on PCCT who subsequently underwent ICA at our tertiary center. The per-vessel sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and diagnostic accuracy(DA) of PCCT were calculated for detecting≥50% or≥70% stenosis with ICA as the reference. The same analysis was conducted in a group of patients with EID-CT. Results: A total of 200 vessels were evaluated involving 4 different coronary segments: left main, left anterior descending artery, left circumflex artery and right coronary artery. The diagnostic performance of PCCT for detecting ≥50% stenosis was as follows: sensitivity 96.67%(91%CI[88.47%-99.59%]), specificity 85.00% (95%CI[77.99% -90.47%]), PPV 73.42%(95%CI[64.99%-80.42%]), NPV 98.35%(95%CI[93.83%-99.57%]), DA 88.50%(95%CI[83.25%-92.57%]). While diagnosing ≥70% stenosis, the sensitivity 100.00%(91%CI[91.96%-100.00%]), specificity 91.03%(95%CI[85.40%-95.01%]), PPV was 75.86%(95%CI[65.60%-83.82%]), NPV was 100.00% (95% CI [97.44% to 100.00%]) and the DA was 93.00%(95%CI[88.53% to 96.12%]). Using EID-CT for the identification of≥50% stenosis the following results were present: sensitivity 96.88%(91% CI [89.16%-99.62%]), specificity 71.32% (95%CI [62.95%-78.75%]), PPV 61.39% (95%CI[54.86%-67.53%]), NPV 97.98% (95%CI[92.51%-99.48%]), DA 79.50%(95% CI[73.23%-84.87%]), whereas identifying vessels with ≥70% luminal narrowing: sensitivity 84.62% (91%CI [71.92%-93.12%]), specificity 81.08% (95%CI [73.83%-87.05%]), PPV 61.11%(95%CI[52.47%-69.11%]), NPV 93.75%(95%CI 88.75%-96.61%]), and 82.00%(95%CI[75.96%-87.06%]) DA were found.
Conclusions: The PCCT scanner provides excellent diagnostic performance in detecting obstructive coronary artery disease, in a per-vessel analysis, furthermore PCCT proved to have better diagnostic accuracy when compared to EID-CT.Funding: MB was was supported by the ÚNKP-23-4-I-SE-23 New National Excellence Program
