Pattern of exercise blood glucose responses and its association with performance parameters in elite athletes
Introduction: Exercise blood glucose levels can be influenced by pre-exercise carbohydrate intake, glycogen stores, and carbohydrate metabolic status, all crucial for athletic performance.
Aims: Our aim was to examine exercise blood glucose changes in athletes during CPET, focusing on response patterns and correlations with performance metrics.
Method: We continued our previous research on exercise glucose responses and focused on athletes who underwent multiple sports cardiology screenings including CPET with 3-point blood glucose measurements. Athletes were categorized into 4 groups regarding glucose changes: increase-increase (II), decrease-increase (DI), increase-decrease (ID), decrease-decrease (DD). For those with multiple CPETs, we compared individual test results.
Results: A total of 901 tests was done on 496 elite athletes (19±5years; male=402), of whom 189 had multiple tests (2 tests: n=100, >2 tests: n=89) resulting in 395 comparative measurements. Sports included basketball(n=96), swimming(n=66), soccer(n=19), other (n=7), water polo (n=1). In comparison of the first two consecutive tests, 106 athletes had similar glucose responses during the two tests. First vs. second exam results: DI 97(51%) vs. 108(57%), II 84(44%) vs. 73(39%), ID 6(3%) vs. 6(3%), DD 2(1%) vs. 2(1%). Comparisons of consecutive tests revealed no association between the changes in peak glucose levels and performance metrics. Greater increase in recovery glucose in the second test correlated with longer exercise time (r=0.19, 95% CI [0.09–0.28], p=0.0001), higher peak lactate (r=0.17, 95% CI [0.07–0.26], p=0.001) and maximal aerobic capacity (r=0.10, 95% CI [0.00–0.20], p=0.042).
Conclusions: Blood glucose response in elite athletes remains consistent across repeated CPETs. Athletes with increasing trend in restitution glucose levels had better performance during the control CPET tests.
