Trends in Voluntary Indoor Smoking Bans in Hungary, 2009–2022: Preliminary Results of a Secondary Analysis

Introduction
One of the public health objectives of the European Union is to reduce the proportion of smokers below 5% by 2040. The voluntary adoption of complete indoor smoking bans in private homes can support this aim, as such rules are associated with lower smoking uptake among adolescents, increased cessation, reduced relapse, and reduced exposure to secondhand smoke. Partial bans are insufficient to reduce indoor exposure; therefore, the WHO recommends the promotion of 100% smoke and aerosol-free environments. Although several interventions have aimed to promote voluntary household bans—especially in families with children—implementation remains limited. Vulnerable groups, including those with lower education or living in deprivation, are less likely to adopt such rules. Moreover, emerging nicotine and tobacco products pose additional challenges. Hungarian data on the trends of indoor smoking bans is lacking.
Aims
To address this research gap, a secondary analysis was conducted using three questionnaire-based surveys from 2009 (Fogarty), 2010 (Eurobarometer 72.3), and 2022 (HAS). Each survey is representative along key sociodemographic dimensions and includes questions on voluntary indoor smoking rules.
Method
Because the surveys used slightly different questions and response options, data were transformed to ensure comparability across surveys, focusing on complete bans on traditional tobacco smoking indoors. Prevalence trends and associations with sociodemographic variables were analyzed using descriptive statistics, Wilson Score Interval calculation, Z-test, cross-tabulation tests, Cochran–Armitage trend tests, and multivariate binomial logistic regression with marginal effects.
Results
The proportion of voluntary smoke-free homes increased significantly between 2009 and 2022 [Z(3) = 7.4, p = 1.3E-13, r = 0.16; 2009: 59.3% (95% CI 57.2%–61.4%), 2010: 63.3% (95% CI 60.3%–66.3%), 2022: 74.7% (95% CI 72.0%–77.4%)]. However, this positive trend is not equally observed across all social groups, with financially deprived populations showing a lower level of increase.
Conclusion
Our findings underscore the need for tailored public health interventions aimed at reducing socioeconomic disparities in the implementation of voluntary household smoking bans.
