Introduction:
Cardiovascular diseases remain the leading cause of global mortality, driven by a combination of metabolic and inflammatory factors. Psoriasis, a chronic inflammatory disease, mainly affecting the skin, has been associated with elevated cardiovascular risk resulting in higher prevalence of major cardiovascular events, due to shared immune-mediated pathways and excessive systemic inflammation. While several studies report beneficial effects of anti-inflammatory biologicals on cardiovascular risk, comparative data remains inconclusive, and the most effective agent has yet to be identified.
Aims:
Our research aims to systematically review and synthesize existing literature on the cardiovascular effects of biological therapies in psoriasis, to identify which biological therapies reduce the incidence of cardiovascular events most effectively.
Methods:
The searches were conducted in three databases (PubMed, Embase, and Cochrane Library) to identify publications reporting major cardiovascular outcomes in psoriatic patients treated with biological therapies (TNF-alpha inhibitors, interleukin-12/23, interleukin-17, and interleukin-23 inhibitors).
Results:
The finalized search strategy yielded 14,932 hits, which were narrowed down to 3,153 records following the title and abstract selection. Data are currently being extracted from 325 full-text articles, including 142 studies on TNF-alpha inhibitors, 63 on interleukin-12/23 inhibitors, 107 on interleukin-17 inhibitors, and 89 on interleukin-23 inhibitors.
Conclusions:
Given the potential cardiovascular implications of psoriasis treatments, synthesizing the current evidence is critical for clarifying possible therapeutic benefits, supporting evidence-based clinical decision-making.
