Introduction: Oral bacteria have been associated with several systemic diseases and studies highlighted its potential role in carcinogenesis. Biofilm is considered an antimicrobial resistance gene reservoir and the oral cavity provides an excellent environment for biofilm formation.
Aim: Our aim was to evaluate the pathogen spectrum and antimicrobial resistance rates of clinical isolates from head and neck infections in the Hungarian population and to estimate the effect of patient-specific influencing factors on antimicrobial resistance (AMR) and the prevalence of multidrug-resistant (MDR) bacteria.
Methods: A total of 3995 bacterial isolates from 1413 patients have been evaluated. Primary diagnoses of the patients were categorized into three major groups: abscesses, necrotizing lesions and surgical site infections of patients treated for malignant tumors. We evaluated clinical isolates from oral- and maxillofacial bacterial infections and compared the resistance patterns to several factors from the medical history of the patients. These factors include age, primary diagnosis that the patients were treated with; comorbidities such as immunosuppressed state and diabetes; oral hygiene; smoking habits; alcohol consumption and previous antibiotic treatment.
Results: Cancer patients had significantly greater percentages of Enterobacter spp., Enterococcus spp., Pseudomonas spp. and beta-hemolytic streptococci than did the other two patient groups. Substantial resistance rates to clindamycin were observed for Prevotella, Streptococcus and Staphylococcus spp. The risk of having MDR bacteria were significantly higher in Escherichia spp., Pseudomonas spp., beta-hemolytic Streptococci and coagulase-negative Staphylococci compared to other species. Those patients that were treated with malignant tumor or underwent immunosuppressive therapy had significantly higher risk for having MDR pathogens compared to other patients.
Conclusion: Our evaluation revealed high percentages of Enterobacterales in patients with oral cancer. Substantial antimicrobial resistance rates, particularly to clindamycin, pose a major concern for treating bacterial infections in the head and neck region.
