Introduction
Literature on fecal microbiota transplantation for fulminant Clostridioides difficile infection (CDI) in adults with hematologic malignancies and chemotherapy-induced aplasia remains limited.
Aims
This study aimed to evaluate the safety and efficacy of pathogen-reduced stool supernatant transplantation in this high-risk population.
Methods
A single-center, non-randomized, single-arm interventional study was conducted in 2024. Recipients with fulminant CDI refractory to high-dose oral vancomycin, intravenous metronidazole, and tigecycline were enrolled during chemotherapy-induced aplasia. CDI severity and classification followed current international guidelines. The intervention consisted of two doses of pathogen-reduced stool supernatant administered via nasogastric tube. The fecal material was derived from an unrelated donor and processed through sieve filtration, centrifugation, and mechanical bacterial filtration to achieve pathogen reduction. Quality control was performed using culture-based methods. Clinical, laboratory, radiological parameters were monitored according to a predefined protocol. The primary outcome was sustained clinical cure; secondary outcomes included survival and the incidence of adverse events at +14 and +30 days post-intervention.
Results
Three adult patients with adverse-risk acute myeloid leukemia, representing various treatment stages and remission statuses, received stool supernatant a median of 8 days following CDI onset. By day +14, all patients achieved sustained cure. At day +30, one patient had succumbed to sepsis unrelated to the intervention, while the remaining two maintained remission. No adverse events directly attributable to the intervention were observed, and follow-up laboratory and imaging studies confirmed resolution of CDI.
Conclusion
In this pilot evaluation, pathogen-reduced stool supernatant transplantation appeared to be a safe and potentially effective treatment option for fulminant CDI in adult patients with hematologic malignancies during chemotherapy-induced aplasia.
