Introduction:
Modern surgical techniques and digital tools have significantly increased the accuracy and success rates of mandibular reconstruction procedures. A multidisciplinary approach extends beyond free flap surgery to include the restoration of dental function through implant-retained prosthetics. The use of 3D-printed, patient-specific surgical guides enables the immediate (primary) placement of implants, which may offer faster and more efficient rehabilitation compared to delayed (secondary) implant placement.
Aim:
The aim of this research is to comprehensively summarize the key aspects of mandibular reconstruction essential for successful dental rehabilitation, to compare the benefits and limitations of primary versus secondary implant placement, and to identify factors that may influence the choice between the two approaches.
Methods:
A literature search was conducted in the PubMed, Embase, and CENTRAL databases, including articles published up to November 2024. Out of 16,543 results, 11,772 articles remained after duplicate removal and were screened by two independent reviewers based on titles and abstracts. A total of 185 studies were selected for full-text review, and ultimately, 23 publications were included in the research. A meta-analysis is being performed based on the statistical synthesis of the eligible studies in accordance with the PRISMA guidelines.
Expected Results:
The study aims to provide objective data on the success and survival rates of primary versus secondary implant placement and the incidence of complications associated with each method.
Conclusion:
If no significant difference is found in the success rates of the two approaches, primary implantation may be preferred in the absence of contraindications, as it reduces treatment time and surgical interventions. Digital technologies offer new opportunities for integrated surgical and prosthetic planning, allowing immediate implantation and a faster, more precise complete rehabilitation. This research may contribute to more effective dental rehabilitation following reconstruction and serve as a practical guide to improve outcomes in future procedures.
