Introduction. Measuring the maximal isometric strength (MIS) of the hip abductor (ABD) muscles is an essential tool for injury prevention in youth soccer. The use of a hand-held dynamometer (HHD), particularly when placed at the ankle, on the lateral malleolus (LM), allows for a more functional assessment with a longer lever arm. However, body position during testing may affect the reliability and sensitivity of the results. This study compares these properties across three commonly used body positions in the literature: side-lying (SL), supine (SP), and standing (ST), using a standardized distal placement of the HHD at the LM. Method. Fifty male soccer players (15.8 ± 1.6 years) from U14, U16, and U18 categories participated in this cross-sectional observational study. Hip ABD strength was assessed using an HHD placed on the LM. Measurements were conducted in three positions: SL, SP, and ST. Each test was performed during the second session of two consecutive weeks. Intraclass correlation coefficient (ICC), standardized typical error of measurement (STEM), coefficient of variation (CVTE), minimal detectable change at 95% confidence (MDC95), and sensitivity (SWC) were calculated. Results. The SL position showed the highest reliability, with ICC values of 0.96 (dominant) and 0.95 (non-dominant), followed by SP (ICC = 0.93 and 0.95) and ST (ICC = 0.92 and 0.94). The lowest CVTE was observed in SL (0.10 and 0.11), intermediate in SP (0.10 and 0.09), and highest in ST (0.17 and 0.14), indicating greater variability in the upright position. MDC95 was also lowest in SL (0.28–0.29 Nm/kg), compared to SP (0.29–0.25 Nm/kg), and significantly higher in ST (0.46–0.40 Nm/kg). Regarding sensitivity, all positions were rated as “marginal” for detecting small changes (SWC0.2). However, SL and SP were rated as “good” for detecting moderate changes (SWC0.6 = 0.27–0.28 in SL; 0.22 in SP), whereas ST showed lower accuracy (SWC0.6 = 0.34–0.37). For detecting large changes (SWC1.2), all positions were considered “good.” Conclusion. To assess ABD MIS using an HHD at the LM, the SL position offers the greatest reliability and sensitivity. The SP position also yields good results, while the ST position is less accurate. Although SL provides better values, it is recommended that sports professionals and researchers use the SP position, as it is easier to implement, more comfortable for the player, and yields similar outcomes. This protocol helps detect real changes in strength and improves injury risk monitoring.
