Introduction: Cardiac allograft vasculopathy (CAV) is a leading cause of long-term mortality in heart transplant recipients. Coronary CT angiography enables non-invasive surveillance, and pericoronary adipose tissue (PCAT) metrics have emerged as markers of coronary inflammation.
Aim: Our aim was to assess longitudinal changes in PCAT characteristics during rapid progression of CAV, a context in which their behavior remains poorly understood.
Methods: We retrospectively analyzed 32 heart transplant recipients (mean age 55.2 years; 12.5% female) undergoing serial CCTA. Inclusion required new ≥50% stenosis in a major coronary branch consistent with CAV. PCAT parameters along the right coronary artery were assessed at diagnosis (CT2) and prior scan (CT1) using Autoplaque. Changes were analyzed with linear regression.
Results: PCAT gradient significantly increased between CT1 and CT2 (+16.3; p<0.001). PCAT attenuation showed a small decrease (−2.7 HU; p=0.028), while PCAT volume remained unchanged (p=0.92).
Conclusions: Rapid CAV progression is characterized by a marked rise in PCAT gradient, with minimal changes in attenuation and no change in volume. PCAT gradient may serve as a sensitive non-invasive marker of pericoronary inflammation in heart transplant recipients.
