Introduction

Pancreatic ductal adenocarcinoma (PDAC), mainly due to its anatomical location, is characterized by clinical symptoms that only occur at the late stage. Consequently, diagnosis is often made when the tumor is already unresectable or metastatic, leading to poor survival rates. Differentiating between malignant and benign pancreatic and bile duct strictures remains a diagnostic challenge. MicroRNAs (miRNAs) are stable, disease-specific biomarkers that may provide high sensitivity in early diagnosis.

Aims

This study aims to identify specific miRNA expression patterns that distinguish malignant from benign lesions in the pancreatobiliary system.

Methods

Samples were collected from over 200 patients undergoing Endoscopic Retrograde Cholangiopancreatography (ERCP) at the National Institute of Oncology, Budapest, Hungary. Based on histological brush cytology, samples were categorized into four groups: (1) benign bile duct, (2) malignant bile duct, (3) benign pancreatic, and (4) malignant pancreatic diseases. Following miRNA isolation, cDNA was synthesized using miRNA-specific primers. Quantification was performed with a real-time PCR-based TaqMan system. Five target miRNAs (miR-196a, miR-21, miR-155, miR-135b, and miR-210) were analyzed, with small nuclear RNA U6 (RNU6B) serving as the internal control. Expression data were processed using the ddCt method and analyzed with ANOVA.

Results

Among the five miRNAs examined, miR-196a and miR-21 demonstrated increased expression levels in malignant processes compared to benign strictures (p-values < 0.05). Although the study is still ongoing, these preliminary results already suggest a diagnostic tendency towards malignancy even in cases where routine cytology may be inconclusive.

Conclusion

The stability and specificity of miRNAs make them promising candidates for molecular diagnostics. Our findings suggest that miR-196a and miR-21 may serve as potential biomarkers for early detection of pancreatic ductal adenocarcinoma, thereby improving patient selection for surgical intervention and promising better survival rates.
