Prospective Study on the Immune Function-Related Cytokine Profile of NSCLC patients treated with First-line Chemo-immunotherapy Combinations

Introduction: The application of chemo-immunotherapy in non-small cell lung cancer (NSCLC) has significantly improved patient survival over the past decade. Aims: Our objective was to assess the immune function-related cytokine profile of NSCLC patients according to long vs short survival.Methods: Between 25 September 2024 and 18 March 2026, we conducted a prospective study at the Reformed Pulmonology Center (Törökbálint) involving 39 patients with NSCLC treated with first-line chemo-immunotherapy combinations. We stratified patients according to long (>10 months) and short (≤10 months) progression-free survival (PFS). We investigated pre-treatment plasma samples from 39 patients using a multiplex ELISA assay targeting 45 immune-related cytokines, chemokines, and growth factors.Results: The study included 20 female and 19 male patients. Adenocarcinoma was diagnosed in 28 patients, squamous cell carcinoma in 9, and NSCLC (not otherwise specified) in 2 cases. High (>50%) tumoural Programmed Death-Ligand 1 (PD-L1) expression was observed in 33 patients (53%). COPD comorbidity was present in 13 patients (33%). The most significant difference between the two groups in pre-treatment plasma was observed for IL-2 (long PFS/short PFS, p=0.0014) and IL-5 (long PFS/short PFS, p=0.0013), concentrations were elevated for short survival. Conclusion: Our findings support the importance of the immune milieu in the efficacy of chemo-immunotherapy in non-small cell lung cancer.
