Patients infected with Helicobacter pylori (HP) may present low-density infections that remain undetected by conventional tests, such as histological examination.

Our aim was to perform HP qPCR on formalin-fixed paraffin-embedded (FFPE) gastric tissue samples from patients diagnosed as HP negative by histology. For qPCR-positive cases, we investigated whether there were any symptoms associated with HP and which symptoms might indicate the need for additional qPCR testing.

A total of 480 FFPE gastric biopsy samples—previously reported as HP negative—were collected. DNA was isolated, and qPCR was performed using our FFPE optimized HPJ primer pair targeting the bacterial 23S rRNA gene. For HP positive samples, clinical symptoms, diagnoses, and histopathological findings were reviewed.

Among the 480 histologically HP negative samples, HP infection was detected in 15 cases (3.13%) by HPJ qPCR. Reviewing these patients’ records revealed gastritis in 12 and erosion in 11 cases. Across all histologically HP negative samples, gastritis was diagnosed in 118 cases, of which 10.17% were qPCR positive, while erosion was found in 81 cases, with 13.58% qPCR positive. Of the 15 HP-positive cases, clarithromycin resistance was detected in five (33%); of these, four were homoresistant and one was heteroresistant.

The presence of Helicobacter pylori on the mucosa is not continuous, and low bacterial density may make histological detection impossible in certain cases. Methods based on extracted bacterial DNA offer a solution to this problem. HPJ qPCR successfully identified HP infection in patients with HP associated symptoms whose biopsies were histologically negative. Therefore, HPJ qPCR can serve as a complementary tool in histologically HP-negative cases, particularly when HP-related clinical findings and/or histological changes are present.
