Comparative One-Year Outcomes of Mineral Trioxide Aggregate and Biodentine in Adult Vital Pulp Therapy

Introduction:
Vital pulp therapy (VPT) has emerged as a conservative alternative to conventional root canal treatment by maintaining pulp vitality. Mineral trioxide aggregate (MTA) is widely regarded as the reference material; however, handling limitations have encouraged the adoption of newer calcium silicate materials such as Biodentine.
Aim: This study evaluated and compared one-year clinical outcomes and coronal discoloration associated with these two materials in adult patients with carious pulp exposure.
Methods:
In this prospective cohort study, 162 teeth were initially assessed, of which 142 met inclusion criteria and received VPT. Treatment modality—direct pulp capping, partial pulpotomy, or full pulpotomy—was determined using predefined clinical guidelines. Teeth were treated with either MTA or Biodentine as the pulp-capping agent and restored definitively when indicated. Clinical success at one year was defined as maintained pulp sensibility or absence of clinical symptoms, together with no evidence of newly developed radiographic pathology. Outcomes were analyzed using ordinal logistic regression, with material type as the primary predictor and age and bleeding duration as covariates.
Results:
Neither patient age (B = 0.004, p = 0.818) nor bleeding duration (B = −0.228, p = 0.119) showed a significant association with treatment outcome. Material choice, however, was a significant predictor: Biodentine demonstrated reduced odds of achieving a more favorable outcome compared with MTA (B = −1.509, SE = 0.589, p = 0.010; 95% CI: −2.664 to −0.355). Conversely, MTA was associated with approximately 4.5 times greater odds of clinical success at one year. Regarding esthetic outcomes, tooth discoloration was observed significantly more frequently in the MTA group (58.1%) than in the Biodentine group (5.9%) (p < 0.001).
Conclusion:
MTA was associated with superior one-year clinical outcomes compared with Biodentine in adult VPT after adjusting for confounding variables. However, Biodentine demonstrated a clear advantage in minimizing coronal discoloration. These findings highlight a trade-off between clinical performance and esthetic outcomes when selecting pulp-capping materials.
